Information you provide
When you submit the contact form or schedule a consultation, you may provide your name, email address, phone number, coverage interest, and a short message. After submitting the form, you may optionally share what prompted your search, your general timeframe, and how you prefer to continue. A unique lead reference connects those optional answers to the original request. If you use the Fixture assistant, the questions and recent conversation context you enter are processed to generate a response. Please do not submit medical records, Social Security numbers, payment information, or other sensitive information through the website or assistant.
How information is used
Fixture may use the information you provide to respond to your request, arrange a consultation, maintain business records, protect the website, and comply with applicable law. Contact information submitted for a specific request will not be treated as consent to unrelated automated marketing.
Service providers
Fixture uses Google Apps Script and Google Sheets to deliver and record website inquiries, Calendly to schedule consultations, and OpenAI to generate assistant responses. Assistant requests are configured not to be stored by Fixture as lead records. If optional site analytics are enabled, Google Analytics may process limited usage events only after the visitor accepts analytics. These providers may process information as needed to perform their services.
Cookies and analytics
Fixture does not intentionally use advertising pixels or session recording. If analytics are enabled, the website asks for permission before loading Google Analytics. Analytics may record general page and conversion events, such as opening the contact form, completing a submission, opening the assistant, or clicking a scheduling link. Fixture configures these events not to include names, email addresses, phone numbers, form answers, chat contents, medical information, or policy details. Visitors may decline analytics and continue using the website.
Retention and security
Information will be retained only as long as reasonably necessary for the purposes described here or as required by law. Fixture will use reasonable administrative and technical safeguards, but no method of transmission or storage is completely secure.
Your choices
You may ask to review, correct, or delete contact information, subject to applicable legal and recordkeeping requirements. You may also ask Fixture to stop non-transactional communications.
